New sites are put in separate file for processing. Access query is used to combine data from 2008 with the latitude/longitude previously collected. New table is created. April 10, 2008 File of new sites is processed in ArcMap with the Centrus Geocoder for ArcGIS. (There were 20.) Latitude and Longitude coordinates were copied into the new table created in the previous step. Table is used to generate XY events. April 11, 2008 Sites with a CENT (centroid) code but a full address were reprocessed using ArcGIS's StreetMap USA address locator and US Address Finder Tele Atlas. See Loc_Code for location method. April, 2008 Addresses that were in counties that had online interactive mapping for their parcels were used to place some points. See Loc_Code for location method. April, 2008 Counties were added with a spatial join. Table of coordinates was generated into personal geodatabase file. April 17, 2008 Metadata from 2007 version of this layer edited for 2008. April 17, 2008 Personal geodatabase file exported to shape file format and copied to M:\GISDATA\Facility April 17, 2008 UPDATES FOR 2009 - Text file dumped from ASPEN system (where facility information is maintained by the Section for Long Term Care) and converted to Access table. A comparison is made between the 2009 data and the facilities from 2008. New facilities are saved into a separate file for processing. A separate table is created combining the facility information from 2009 to the latitude, longitude and location codes (loc_code) from 2008 (common column of unique values = FAC_ID) to create a working temporary table of all 2009 facilities February 2009 New facilities for 2009 (there were 17) were geocoded using the software Centrus Geocoder for ArcGIS. Locations that were only matches to a centroid were checked using web sources (maps.google.com, the facilities own web page, county assessor databases were available) Latitude and Longitude values were updated for the new locations. Location codes (loc_code) were modified to reflect the type of match that was achieved. Latitude, Longitude and Loc_Code were copied and pasted into the missing fields in the working full table of 2009 sites. Table was then displayed as XY events. Events were then exported to personal geodatabase. A spatial join was made to the county boundaries to populate the county name. February 2009 Metadata from 2008 was exported to text and updated for 2009. Metadata in text format was then imported back into the personal geodatabase. The personal geodatabase was then exported to shape file format. The shape file was copied to M:\GISDATA\Facility. (The previous year's layer was copied to M:\GISDATA|archiveFacility and renamed nurHome08.shp. February 2009 Metadata imported. A social model of care includes long-term care services based on the abilities, desires, and functional needs of individuals delivered in a setting that is more home-like than institutional and which promotes the dignity, privacy, independence, and autonomy of the individual. This type of facility may accept or retain residents who are able to evacuate the facility with minimal assistance. A licensed Nursing Home Administrator is required. This level of care also provides oversight for storage, distribution, or administration of medications; and health care supervision under the direction of a licensed physician, and consistent with a social model of care. A social model of care includes long-term care services based on the abilities, desires, and functional needs of individuals delivered in a setting that is more home-like than institutional and which promotes the dignity, privacy, independence, and autonomy of the individual.
