Some facilities, such as VA hospitals or mental health facilities are not regulated by the Department of Health and Senior Services. These facilities are not included.
Geographic locations were created using Sagent Geocoding software. Quality Control by Geographic Resources Center 8/11/2003 to refine locations provided from geocoding.
